To add a new user to your production database, please have a listed Eclipse Supervisor (usually the Accountant) complete the below form:

Please bookmark this link, the form can be used every time you have an amendment to your users.
Important, this form can only be completed by an existing Eclipse Supervisor. The email address of the person that completed the form must match what TPH has on file as a Supervisor for your database.
Once the form is completed it will automatically be sent to TPH Support and you will be notified once the new user(s) have access.
